What is IVF & ICSI?

IVF (In Vitro Fertilization)

Eggs are collected from the ovaries and fertilized with sperm in the lab. The process involves retrieving mature eggs from a woman's ovaries and fertilizing them with sperm outside the body, in a laboratory dish.

ICSI (Intracytoplasmic Sperm Injection)

A single healthy sperm is directly injected into an egg under a microscope, increasing the chances of fertilization. The fertilized eggs, called embryos, are then transferred into the woman's uterus, where they may implant and develop into a pregnancy.

IVF & ICSI Treatment Steps

Step 1: Initial Assessment & Planning

  • Medical history review and diagnostic tests
  • Creating a personalized treatment plan

Step 2: Ovarian Stimulation

  • Baseline ultrasound to check follicle count
  • Daily hormonal injections (FSH/LH) for 8-12 days
  • Medications to prevent premature ovulation
  • Regular monitoring via ultrasounds and blood tests
  • Trigger injection for final egg maturation

Step 3: Egg Retrieval

Important Timing: Egg retrieval is performed 34-36 hours after the trigger injection. This timing is critical for successful retrieval of mature eggs.

  • Performed under sedation/anesthesia
  • Ultrasound-guided needle aspiration of follicles
  • Goal: 8-15 high-quality mature eggs
  • Minimizing risk of OHSS

Step 4: Fertilization

  • IVF: Eggs cultured with sperm in dish
  • ICSI: Single sperm injected directly into egg
  • ICSI increases fertilization chances

Step 5: Embryo Culture

  • Embryos cultured for 3-5 days
  • Day 5 embryos called blastocysts (100-250 cells)
  • Fresh transfer or freezing for future use

Step 6: Embryo Freezing (Vitrification)

  • High-quality embryos can be frozen for future use
  • Vitrification process prevents ice crystal formation
  • Frozen embryos can be stored for many years
  • Allows for multiple transfer attempts from one retrieval cycle
  • Reduces risk of ovarian hyperstimulation syndrome

Step 7: Embryo Transfer

  • Uterus prepared with hormones
  • Painless procedure without sedation
  • Ultrasound-guided catheter placement
  • Usually 1-2 embryos transferred

Step 8: Luteal Phase Support

  • Progesterone supplementation
  • Brief rest, avoiding strenuous activity
  • Beta-hCG test 10-14 days after transfer
